Dear candidates!
Citizens of Ukraine are constantly invited for service in the ranks of the National Police. One of the important stages of selection is passing the medical (military-medical) commission. We provide a list of documents required to pass the M (MM) C for candidates employed in the units of the National Police:
- Referral to the commission.
- Passport, military ID card (military registration certificate). In the military card (conscription certificate) there must be a mark on the registration and suitability (unsuitability), stamped. In the presence of the mark "unfit" for M (MM) C provide medical documents confirming the diagnosis within the specified article.
- Extract from the medical records at the place of residence for the last 5 years (form 027/0), certified by the seal of the medical institution.
- A medical description or excerpts from medical records of military units (for former servicemen who have been discharged from service within the last year).
- Extract from the order of discharge from the police, AFU, SBU, other military formations (for former police and military personnel),
- Copy of resolution of VVK, certificate of illness (for persons who were discharged from service due to health condition).
- Certificate from the TB dispensary, certified by the seal of the medical institution.
- Certificate of previous vaccinations (form 063/0), certified by the seal of the medical institution.
- Certificate of drug treatment facility at the place of residence (form 140).
- Certificate of neuropsychiatric institution at the place of residence (form 122).
- Fluorography (X-ray) with a description, made for the last year, certified by seal and stamp of medical institution.
- Electrocardiogram (ECG), with description and seal, made within the last month.
- Results of a general blood test with a leukocyte formula, within the last month.
- Results of a blood sugar test done within the last month.
- Blood type, Rh factor.
- Results of urinalysis performed within the last month.
- Biochemical blood tests (bilirubin, cholesterol, protein, thymol, AST, ALT) in the past month.
- Results of blood tests (Wasserman reaction).
- Blood test results for Hepatitis B, C and HIV within the last month.
Please note:
1) correctly fill in the boxes of the referral for M (MM) C;
2) be sure to indicate the area where the M (MM) C is located, indicate what position the candidate is employed for;
3) all tests are valid for one month before the medical examination in M (MM) C;
4) all tests must be stamped and stamped by the medical institution;
5) if necessary, you can be further examined in the hospital of the Main Department of the Ministry of Internal Affairs of Ukraine in Kirovograd region.
